Failure to comply with the elements noted and described below can result in the delay of your installation and increase the cost of installation. Once you have read each item, if you are in compliance with it, please mark agree on that item. If there is a discrepancy or it is not complete, please mark "needs attention", and add notes that describe the issue. Once completed, please add your location name, your name or person responsible, current date and your signature and submit.

If there are discrepancies or incomplete items, it will be required that this form is completed again with all points covered and agreed to prior to the installation team leaving for your flooring installation. This form must be completed and submitted within 14 days of the proposed installation date.
